Senior Employee Engagement Specialist at Responsive Health Management Inc.

Responsibilities
  • Develop and modify plans of care in response to residents’ individualized health care needs and change in health status.
  • Liaise with other health care personnel, agencies, and families.
  • Maintain good relationships with medical/support/home staff, residents, families, and the general public.
  • Supervise all activities on the unit and take charge as required.
  • Monitor performance and complete performance evaluations for PSW’s and RPN’s.
  • Direct, assign, and evaluate the work of PSW’s.
  • Utilize leadership skills to identify problems and solve issues on the unit.
  • Ensure care is delivered consistently with the resident Bill of Rights.
  • Understand quality indicators used by the Home and participate in improvement of indicators.
  • Order and ensure nursing supplies and equipment are properly and efficiently used.
  • Actively listen to residents and family concerns and take appropriate actions to resolve issues in accordance with Home policies.
  • Conduct unit rounds at the beginning and end of shift.
  • Process physician orders and administer medications and treatments in compliance with Pharmacy policies and nursing standards; document and report all errors and omissions.
  • Maintain accurate and complete records of nursing observations and care in compliance with legislative requirements and Home policies.
  • Assess changes in health status and report changes to Substitute Decision Maker (SDM) or POA and interdisciplinary team.
  • Complete health care assessments (RAI‑MDS and other assessments), develop and update plan of care for each resident in collaboration with the resident, SDM, and interdisciplinary team.
  • Demonstrate understanding of the Ministry of Long‑Term Care mandatory reporting, critical incidents, and follow steps outlined in Home policies.
  • Perform other duties as assigned and/or indicated in Job Task Inventory.
Qualifications
  • Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, holding the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60.
  • Experience in a long‑term care setting is an asset.
  • Ability to work with and relate to interdisciplinary team members.
  • Well‑developed written and oral communication skills.
  • Knowledge of RAI‑MDS – AIS certified; if not, must complete successfully within one (1) year of being hired.
  • Demonstrate a focus on people’s feelings when interacting and the ability to turn a task into positive social interaction.
  • Undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  • Provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
  • Submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X‑ray (within the last year), as required by Public Health.
