Patient Care Coordinator - State College
Drayer Physical Therapy, a brand partner of Upstream Rehabilitation, is seeking a Patient Care Coordinator to join our team in Port Matilda, PA.
As an entry‑level office role, you will maintain pleasant and consistent daily operations of the clinic, providing excellent customer service and multitasking across multiple computer programs.
What is a Patient Care Coordinator?- Responsible for maintaining pleasant and consistent daily operations of the clinic.
- Exhibits excellent customer service skills.
- Multitasks across multiple computer programs each day.
- Greets everyone who enters the clinic in a friendly and welcoming manner.
- Schedules new referrals received by fax or telephone from patients and physician offices.
- Verifies insurance coverage for patients.
- Collects patient payments.
- Maintains an orderly and organized front office workspace.
- Other duties as assigned.

The Patient Care Coordinator - I (PCC-I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures and support the mission, vision, and values of Upstream Rehabilitation.
Responsibilities- Collect all money due at the time of service.
- Convert referrals into evaluations.
- Schedule patient visits.
- Provide customer service.
- Create an inviting clinic atmosphere.
- Make all welcome calls.
- Monitor and influence arrival rate through great customer experience.
- Practice Management:
- Manage schedules efficiently.
- Manage document routing, personal overtime, non‑clinical documentation, deposits.
- Manage caseload, D/C candidate, progress notes, and insurance reporting.
- Monitor clinic inventory.
- Training:
- Attend required training with Territory Field Trainers (TFT) for Raintree and other business process updates.
- Complete quarterly compliance training.
- High School Diploma or equivalent.
- Strong communication skills and ability to relate well to Business Office and Field leadership.
- Ability to multitask, maintain organizational detail, meet deadlines, and work with little supervision.
- Team member with efficient time management and presentation skills.
- Work in a comfortable indoor environment with exposure to office noise.
- Full‑time position Monday–Friday, 40 hours per week, with occasional evenings, weekends, or additional hours as needed.
- Sedentary work with frequent computer use and occasional light lifting (0–20 lbs.).
- Constantly sit or stand as needed while communicating and using office equipment.
- Must view computer screens and read items on screens.
- Must comprehend information provided and make judgment calls as required.
