We are looking for a compassionate Licensed Practical Nurse (LPN) to join our Skilled Nursing team. As an LPN, you will deliver nursing care to our residents in a warm, comfortable, home‑like environment, while building long‑lasting relationships with both associates and residents.

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in resident behavior and health to the Director of Nursing, physician, family, RCAs, and Health Center Administrator per state regulations.
• Direct and supervise daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants.
• Coordinate care needs with community providers via an effective case‑management process.
• Effectively balance service demands with supporting resident independence.
• Recognize and assist in the prevention of elder abuse, neglect, and exploitation and report to appropriate sources per state regulations.
• Respond to emergencies and the personal emergency response system and know when to call for backup.

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program per state requirements.
• Minimum of 1 to 2 years of experience working as a nurse (preferred).
• Previous experience working with the elderly in a paid or volunteer position is highly desirable.
