Registered Nurse

We are the largest provider of primary health care services in Fairfield County and are looking for dedicated professionals to join our team. With multiple locations in Bridgeport, Stratford, and Stamford, our mission is to be a lifelong health care partner, dedicated to achieving optimal wellness for the communities we serve.
We are adding a full‑time Registered Nurse to our Stamford clinic. The position includes one evening shift until 7:00 pm.
Responsibilities- Promote and restore patients' health by implementing the nursing process; collaborate with physicians and multidisciplinary team members; provide physical and psychological support.
- Advance patient independence by establishing goals and teaching self‑care skills.
- Ensure quality of care by adhering to therapeutic standards; measure outcomes against goals; adjust care as needed.
- Report clinical and operational issues to the Practice Manager, Director of Nursing, or Nurse Manager for immediate response.
- Collaborate with managers to keep staff informed of changes that need implementation.
- Coordinate care with providers and provide appropriate clinical support.
- Resolve patient concerns and/or complaints using multidisciplinary team strategies.
- Monitor tracking systems for patient results; ensure referrals/images are processed and tracked per Optimus policy.
- Assist the Nurse Manager in overseeing tasks of the clinical staff and timely completion.
- Valid, current RN License to practice in the State of Connecticut.
- CPR certification required upon hire and recertification bi‑annually.
- Associate’s Degree from an accredited registered nursing program approved by the State of Connecticut Department of Public Health;
Bachelor’s Degree preferred. - Minimum of one to two years of RN experience in a community health setting.
- Excellent spoken and written communication skills.
- Excellent leadership and motivational skills.
- Ability to build relationships with colleagues at all levels.
- Proficient in using EMR; knowledge of Microsoft Word, Excel, and PowerPoint.
- Bi‑lingual English/Spanish is helpful.
- Strong critical thinking, independent judgment, and ability to manage multiple priorities under time pressure.
