Enhanced community services will come together to support the person to stay safe and independent in the place they call home. The successful post holder will be, in conjunction with the Senior District Nurse, responsible for the day-to-day management of a team of skilled healthcare workers.

This includes the education, support and development of staff and the mentorship of students during placements.

You will supervise and support junior staff to identify areas for their development and work alongside them to coach and mentor. You will be a positive and aspirational role model.

You will be undertaking full nursing assessments of patients, including those with multiple pathologies and complex health and social care needs. This will include the monitoring of medication regimes, the provision of aids and equipment to patients, ensuring that both infection control and health and safety issues are maintained. This will also include liaison with the local Multi-Disciplinary Teams and other CSH services who come together to support our patients within their own homes.

The successful applicant will be part of the Spelthorne Locality Community Nursing Team and will be responsible for the assessment, planning, implementation and evaluation of individualised, holistic nursing care to our patients living within a defined geographical location within the community. You will be responsible for the delivery of care and for managing service provision in accordance with national and local priorities, organisational policies, procedures and guidelines and in accordance with the Nursing and Midwifery Council.

Excellent communication skills are vital to ensure that strong, effective working relationships are nurtured which include those with GPs, Adult Social Services, acute/community hospital colleagues, Borough councils and the voluntary sector. Together these services will come together to form the 'Neighbourhood' approach to our patient care, which puts the patient at the centre of all that we do.
