Clinical Therapist II (Crisis) - Per Diem — SOUTHERN OCEAN MEDICAL CENTER, Manahawkin, New Jersey.
Requisition # Shift: Evening. Status:
Per Diem.
The Clinical Therapist II treats patients by utilizing a variety of modalities as a member of the multidisciplinary clinical treatment team. Also provides psychological/social interventions that support patients and families relating to behavioral health, levels of care, hospitalization, disease and illness and wellness and recovery.
Responsibilities- Provides a comprehensive assessment of a patient s psychosocial needs, develops treatment plans and treatment plan reviews based on findings and according to department policy.
- Provides effective behavioral health treatment and education to patients, including assessment or therapy (individual and/or group), and shares and interprets information to staff, families, and/or other approved agencies.
- Completes all documentation and written reports in a timely fashion in accordance with department and hospital policy.
- Provides services to patients in accordance with licensure regulations defined by the respective licensure board.
- Utilizes information about patient needs, development, and evidence-based treatment options to develop patient-specific treatment plans.
- Provides comprehensive case management services/assessment as warranted.
- Assesses Social Determinants of Health (SDOH) and provides information, education and/or referrals as appropriate to meet identified patient needs.
- Collaborates with the patient (with their consent), family, and other support persons involved in the patient s care to develop a comprehensive discharge plan.
- Reassesses treatment plan on an ongoing basis, evaluates progress, and revises the plan when clinically indicated.
- Maintains protected patient health information in compliance with laws and policies.
- Assists other behavioral/medical team members in responding to emergencies.
- Has working knowledge of community agencies and resources and refers patients when appropriate.
- Assists with interview and orientation of new staff members; supervises social work/counseling students on field placement.
- Identifies the needs of the patient population served and adapts care to age, culture, language, and accommodations; communicates at patient/ caregiver levels.
- Lifts, pushes, and pulls physical loads as required by the job; stands for extended periods as needed.
- Adheres to the Medical Center s organizational competencies and receives supervision as required by licensure.
- Performs additional duties as assigned.
Note:
Department/program may have specific requirements based on level of care and patient population; these will be reviewed during interview and at hire.
- Master s degree in Social Work (MSW) or Master s degree in Counseling or a related field.
- Minimum of two years of post-licensure work experience in the same field.
- Ability to perform psychosocial assessments and develop treatment plans.
- Knowledge of crisis intervention theory and its application, disease management and differential diagnoses, and treatment techniques.
- Ability to work as part of a team and build collaborative relationships.
- Excellent follow-through and attention to detail; strong written and verbal communication skills.
- Proficient computer skills (e.g., Microsoft Office and/or Google Suite).
- Professional Counselor License or NJ Licensed Clinical Social Worker or Marriage and Family Therapy, Professional Counselor.
EPS Department Specific Certification:
Crisis Prevention & Intervention (CPI) certification, or completion required during new hire orientation.
