• Collaborate with team members and multi-disciplinary partners from various agencies to support vulnerable adults.
• Ensure the voices of individuals and carers are central to decision-making processes, including conducting Mental Capacity Assessments and making Best Interest decisions.
• Manage Court of Protection applications and represent cases at hearings when required.
• Conduct safeguarding enquiries and interventions for issues such as self-neglect, in compliance with legal standards.
• Promote independence through re-ablement strategies and facilitate effective communication between families, carers, and other involved agencies.
