Licensed Practical Nurse (LPN) – Up to $32/hr

• Patient Assessment and Care Planning:
  Assist in assessing patients’ health status and contribute to individualized care plans.
• Direct Patient Care:
  Provide safe, effective nursing care for multiple patients within the facility’s program.

  Implement education initiatives based on established teaching plans.
• Nursing Scope of Practice:
  Perform delegated tasks consistent with education and within scope, use a problem‑solving approach for clinical decisions.
• Care Coordination and Monitoring:
  Oversee onsite and offsite scheduling with the care team, ensure follow‑up care including telemedicine appointments.
• Quality Improvement and
  
  Collaboration:
  
  Participate in quality improvement initiatives and patient care committee meetings; coordinate with regional medical and nursing directors for seamless care.
• Documentation:
  Monitor CARES and ensure proper coordination, perform additional nursing duties as assigned to support outcomes.

• Educational Background:
  Graduate from an accredited Licensed Practical Nursing program.
• Licensure and Certification:
  Current LPN license in the state of employment and active CPR certification.
• Knowledge:
  Familiarity with state laws governing LPN practice; successful completion of initial and annual clinical competencies.
• Other Requirements:
  Ability to pass background check, security clearance, and drug screening.
