As the Physical Therapist, you will use discretion and independent judgment to provide age-appropriate services for hospice, including the following: comprehensive assessment, direct treatment to individuals, development of a home exercise program, patient/family instruction, participation Interdisciplinary Group, evaluation and selection of adaptive equipment, and documentation of services according to hospice standards.

Primary Responsibilities
  • Initiates physical therapy evaluation according to physician orders and Plan of Care for the purpose of symptom control or to enable the patient to maintain activities of daily living and basic functional skills
  • Documents in the patient's clinical record each visit made to the patient and gives report including patient's reaction to treatment and any changes in the patient's status including comfort level and follow-up to physician and Administrator using the Care Management process
  • Participates in the Interdisciplinary Group and provides applicable input for the patient's Plan of Care as needed
  • Instructs and informs participating members of the Interdisciplinary group, the patient, the physician and the family functional limitations and progress towards goals as by the Plan of Care daily or no less than two weeks' time
  • Prepares a written discharge summary when physical therapy services are discontinued with a copy retained in the patient's clinical record and a copy forwarded to the patient's physician conducted upon discharge from the physical therapy service
  • Instructs, when appropriate, patient, family and other members of the hospice team in certain phases of physical therapy in which they may work with the patient

Required Qualifications
  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
  • Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
Preferred Qualifications
  • Proven ability to manage multiple tasks simultaneously
  • Proven ability to work independently
  • Proven good communication, writing, and organizational skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $35.00 to $62.50 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
