Position details- Position Type:
Full-Time - Shift: 1st Shift - no weekends
- Work Schedule:
M-F 40 hrs
- Collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
- Performing initial intakes, assessments, and timeline summaries.
- Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
- Providing individual and group treatment and support according to each individual’s/family’s needs.
- Supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.
- Bachelor’s degree in Social Work, Psychology, or another human service-related field required.
- Will be required to complete clearances and background checks, at Merakey’s expense.
- Valid PA driver’s license with a clean driving record required.
- 1 year of experience in the behavioral health field preferred.
- Candidate must be willing to travel.

The Case Manager will be responsible for:
- Collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
