Job Description

Peace Health is seeking candidates for a Manager of Care Management position at Sacred Heart Medical Center in Springfield, Oregon. In this full‑time position you’ll manage overall operational leadership and department functions of the inpatient care management department. Location

Sacred Heart Medical Center is a 350‑bed, Level II trauma center located along the McKenzie River in Springfield, Oregon. The city is an hour from the Pacific Ocean and two hours south of Portland, offering outdoor adventures, cultural activities, and proximity to the University of Oregon.

Key Responsibilities
  • Provides direction, leadership, management and oversight of the department.
  • Strategic and operational planning, implementation and evaluation to ensure effective patient care management.
  • Participates and leads process improvement projects to address system‑wide delays and improve quality clinical coordination and patient management.
  • Supervises staff and gives technical direction, monitors and prioritizes workflow.
  • Manages the selection, training and development of staff, including performance assessments.
  • Works with physicians, medical social workers, nursing unit managers and educators to provide effective patient care coordination.
  • Ensures department compliance with all regulatory standards and leads staff to a culture of safety, continuous improvement and quality patient care.
  • Participates in the budgeting and budget monitoring processes.
  • Performs other duties as assigned.
Qualifications
  • Required:

    Bachelor of Science in Nursing degree.
  • Required:

    Current Oregon RN license.
  • Required:

    Strong inpatient care management experience in a hospital setting.
  • Required:

    Progressive leadership experience.
  • Required:

    Must obtain “Certified Case Manager” within 2 years of hire date.
  • Preferred:
    Minimum of 2 years of management experience in a healthcare organization.
  • Preferred:
    Solid leadership background.
Skills
  • Strong leadership, needs assessment, program development, project management and organizational alignment skills (required).
  • Supervisory skills (required).
  • Strong problem‑solving and decision‑making skills (required).
  • Strong communication skills, both verbal and written (required).
  • Strong training and facilitation skills (required).
  • Proficient computer skills, including MS Office applications (required).
Working Conditions

Lifting

  • Consistently operates computer and other office equipment.
  • Exerts up to 10 pounds of force occasionally or negligible force frequently or constantly to lift, carry, push, pull or otherwise move objects.
  • Work is primarily sedentary.
Environmental Conditions
  • Predominantly operates in an office environment.
  • Some time spent on site in a medical/hospital setting.
Mental/Visual
  • Ability to communicate and exchange accurate information.
  • Requires close visual acuity to perform activities such as preparing and analyzing data, transcribing, and reading a computer terminal.
