The Home Health RN Case Manager provides one‑on‑one care and skilled services to patients in their own homes, contributing to the individual’s overall quality of life. Provides in‑home evaluation, direction and application of skilled nursing services to complement the patient’s overall plan of care. Case management ensures the overall patient condition is cared for by providing appropriate education and resources to the patient and caregivers, allowing them to manage their conditions and remain at home.

• Required:
  
  Graduate of an accredited nursing program or NLN approved program
• Preferred:
  Bachelor’s Degree in Nursing

• Preferred:
  Minimum one (1) year experience as a professional nurse within the last three (3) years
• Preferred:
  Previous nursing experience

• Ability to provide reliable transportation as required by nature of position
• Excellent verbal and written communication skills
• Demonstrate effective leadership abilities
• Exhibits valuable time management skills
• Strong critical thinking/problem‑solving skills
• Flexibility and ability to work in a multi‑tasking environment

• Required:
  
  RN license active in the state of Missouri

  BLS must be obtained within 90 days
