Program and Outreach Director

Overview

The Bungalows at Springfield East is seeking an Activities Director (Program and Outreach Director) to join their team!

The Program and Outreach Director reports directly to the Executive Director.

• Develop, lead and oversee a resident-centered activities and outreach program that promotes and integrates the Phoenix Senior Living philosophy to incorporate wellness in all aspects of the seniors’ lives we serve.
• Ensure all dimensions (emotional, sense of purpose, physical, social, intellectual and spiritual) are incorporated into the services we provide.
• Assist in recruiting, hiring and training of the Assistant Program and Outreach Director.
• Participate in the Personalized Service Plans (PSP).
• Refer to the Resident Profile and addendum for every new resident.
• Provide a variety of appropriate and personalized activities that incorporate all dimensions of wellness and are available throughout the day and evening with resident care associate participation.
• Maintain the PSL Activity & Program Guidebook, including tracking daily which residents exhibit interest and participate in which activities.
• Use residents’ demographic profile information to organize small groups or clubs addressing similar needs and preferences in a small group/club setting.
• Recognize changes in resident condition and take appropriate action.
• Assist residents to and from the activity rooms when necessary.
• In memory care, assist residents to and from Life Skills and other normalizing life-enriching activities and routines and assist with engagement.
• Assist in maintaining an inventory of activity and programming supplies, games, programs and crafts.
• Schedule, communicate, assist with facilitation and record monthly resident Council.

• Education:
  
  Associate degree in Business Administration, Human Resources Administration, Gerontology or other related field preferred.
• 1 – 3 years’ experience in senior living/skilled nursing activity & event planning, which may include moderate to large event planning responsibilities outside the senior living industry.
• Must have demonstrated leadership capabilities.
• Knowledge and experience in the Assisted Living industry and Dementia care preferred.

  Total hours/day: 4 – 6.
• In an 8 hour workday, the associate may sit;
  Hours at one time: 0 - 2;
  Total hours/day: 2 – 4.
• In an 8 hour workday, the associate may drive; 30-60 minutes, 1 - 2 times a week.
• Associate will support/assist (Maximum lbs.): 50 lbs. Frequency;
  Occasionally: 150 lbs.
• Associate will lift/carry (Maximum lbs.): 40 lbs. Frequency;
  Occasionally: 70 lbs.
• Height of lift: 3 – 4 feet;
  Distance of carry: 30 yards.
• Hands use for repetitive tasks:
  Simple grasping, pushing, and pulling; fine manipulation.
• Body mechanics:
  Bend frequently;
  Squat frequently;
  Kneel frequently;
  Climb frequently;
  Reach occasionally (up to 3 feet).
