Care Coordinator; Springfield

HEALTHY COMMUNITIES. COMPASSIONATE CARE. Ozarks Community Hospital in Springfield, MO has an immediate opening for a full‑time Care Coordinator within our Evergreen Clinic. This role may require occasional travel to surrounding clinics as needed.

Under the PCHH Director, this position will be responsible for ensuring that Medicaid beneficiaries are successfully enrolled in the MO DSS Health Home. They will track OCH’s Health Home patient’s ER utilization daily and follow patients’ Medicaid eligibility/coverage. They will work cooperatively with a patient’s care team to maximize outcomes. Has the ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.

• High school diploma or equivalent.
• Minimum of one (2) year’s medical office experience preferred.
• Previous experience with Medicaid coverage preferred.
• Completion of medical terminology course or equivalent experience preferred.
• Occasional travel to surrounding clinics as needed.
