Registered Nurse – Patient Services
Full‑time on‑site position in Springfield, MA (Requisition Number: REGIS
001017).
Sign‑on bonus available.
Responsibilities- Provide coordinated care to adult and pediatric patients, collaborating with patients and interdisciplinary care team members to ensure comprehensive, patient‑centered outcomes.
- Conduct comprehensive health assessments, record medical histories, complete medication reconciliations, evaluate patient conditions, and contribute to individualized care and treatment decisions.
- Work effectively in both clinic and urgent care settings, utilizing critical thinking skills to make sound clinical judgments and prioritize patient needs in a fast‑paced environment.
- Perform nurse visits, providing clinical assessments, patient education, and nursing interventions following established clinical protocols and scope of practice.
- Educate patients and their families about health conditions, treatment plans, and preventive care, proactively addressing gaps in care.
- Provide team‑based nursing care within assigned pods, collaborating with providers and support staff to ensure coordinated and efficient patient care.
- Administer prescribed medications, vaccines, and treatments following approved clinical protocols and standards of nursing practice; ensure accurate and timely documentation in the electronic medical record.
- Collaborate with physicians, other nurses, and healthcare professionals to coordinate patient care and ensure smooth transitions.
- Respond promptly to all patient‑related telephone calls and in‑basket messages, ensuring timely communication and continuity of care.
- Participate in health center outreach initiatives and community activities to promote public health and strengthen community engagement.
- Maintain strict confidentiality of all patient and health center information in accordance with HIPAA and organizational policies.
- Other duties as assigned.
- Excellent clinical assessment and critical thinking skills, with a solid understanding of disease processes and age‑specific development.
- Demonstrated ability to deliver high‑quality nursing care in collaboration with physicians and support staff.
- Culturally sensitive, with a strong interest in serving diverse patient populations.
- Ability to establish and maintain effective working relationships with other employees, individuals seeking assistance, representatives of outside agencies, and the general public.
- Proficient in electronic health record (EHR) systems and familiar with outpatient and ambulatory care best practices.
- Excellent verbal, written, and interpersonal communication skills.
- Flexible, professional, and reliable, with the ability to adapt to varying patient needs.
- Committed to patient‑centered care, customer service, and ongoing professional growth.
- Bilingual preferred (Spanish, Russian, or Vietnamese).
- Current and unrestricted RN license in Massachusetts.
- Current BLS CPR certification.
- Minimum of one year of nursing experience.
- Required to cover extended clinic hours, including evenings and weekends, on a rotating basis at any Caring Health Center location.
Work is performed in a busy outpatient clinic environment with direct patient contact. Requires frequent standing, walking, bending, and lifting. Involves the use of computers, phones, and other standard medical office equipment. Must be able to respond quickly in urgent or emergent situations.
