The Center for Human Development (CHD) is seeking a Registered Nurse – RN for our Adult Community Clinical Services (ACCS), located in Springfield, MA. ACCS is funded through the Department of Mental Health and offers a multidisciplinary outreach approach for individuals in need of psychiatric and other services, living independently in the community. The role involves active engagement including assertive outreach, assessment and treatment planning, skill building, symptom management, addiction treatment, and health monitoring.

YOUR ROLE AS A REGISTERED NURSE:ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The team collaborates to provide person-centered, evidence-based services aligned with treatment goals. The RN coordinates activities that promote the health of Persons served, provides direct support for needs assessment, participates in medical reviews, offers counseling on health and psycho-physiological well-being, and liaises with medical and community providers.
This is an administrative RN role.
- Bachelor's degree preferred
- MA Registered Nurse License required
- Bilingual (English/Spanish) candidates required
- Two years of experience in mental health and/or substance use preferred
- MAP experience preferred
- Residential experience preferred
- Acceptable driving record

Annual Salary: $80,000
