Salary/Status:
Exempt
- Current Massachusetts NP licensure and/or eligibility for certification.
- Current Mass CSR and DEA licenses.
- Certification in Family Practice, Adult, or Pediatric.
- Ability to collaborate with a multi-disciplinary team, including physicians, nursing staff, social workers, and case managers.
- Previous experience in ambulatory care and/or public health is preferred.
- Demonstrate proficiency in assessment and delivery of family planning services to a diverse population.
- Desirable:
Fluency in Spanish, Arabic, Russian, Nepali, Vietnamese, or another language served at CHC. - Special expertise in provision of health care to underserved, culturally diverse populations.
- Conduct comprehensive patient assessments.
- Identify potential and actual health and health-related problems.
- Evaluate patient responses associated with problem area(s).
- Diagnose acute and chronic illness.
- Develop and implement treatment plans that include pharmacologic and non-pharmacologic components.
- Include the patient and family as active participants in the treatment plan.
- Focus NP/Patient interaction on wellness, health promotion, disease prevention, and self-care.
- Offer comprehensive clinical family planning services when appropriate.
- Review patient charts and ensure appropriate treatment plans are developed and followed.
- Complete charts on the same day of care or no later than 72 hours after the visit.
- Ensure chart completion meets risk management and chart audit requirements.
- Collaborate and consult with CHC physicians and other providers; refer patients to appropriate specialists and other practitioners and facilities.
- Cooperate with CHC administration to monitor and improve patient care quality by evaluating patient outcomes.
- Maintain updated Nurse Practitioner guidelines.
- Participate in peer review of charts.
- Maintain all CME/CEU, Board and Licensing requirements of state and national governing boards.
- Comply with Caring Health Center’s compliance standards, including its Standard of Conduct, Compliance program, and policies and procedures.
- Include compliance as part of regular performance evaluation.
- Require ability to use a computer; work at a computer workstation; view a CRT; and have some knowledge of a keyboard.
