The Center for Human Development (CHD) is seeking a Bilingual Outreach Worker for our Adult Community Clinical Services (ACCS) in Springfield, MA. ACCS is funded by the Department of Mental Health and provides a multidisciplinary outreach approach for individuals in need of psychiatric and other services living independently in the community. Services include active engagement, assessment, treatment planning, skill building, symptom management, addiction treatment, and health monitoring.

Your Role as Direct Care:ACCS teams consist of licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The team collaborates to provide person-centered, evidence-based services aligned with treatment goals. The Direct Care Staff helps participants acquire skills for independent living, working within a team that emphasizes communication, support, and accountability.
Requirements:- At least one year of experience working with the target population (preferred)
- Bachelor’s degree from an accredited institution (preferred; required for DC II or with additional experience)
- High School diploma
- Fluent in English and Spanish, with the ability to speak, read, and write in both languages
- Assist participants with daily activities such as hygiene, meal prep, housekeeping, laundry, and money management
- Transport participants for medical appointments, shopping, interviews, and other services
- Administer medication following MAP policies and procedures
- Maintain records including assessments, progress reports, incident reports, and disbursements
- Participate in recreational activities like dining out, sports, and movies
- Contribute to the development of treatment plans
Pay Rate: $20/hr

Qualifications:
High School diploma required;
