Summary DescriptionThe Office Administrative Nurse performs administrative duties to assist the home health team managers, patient care coordination, and facilitates patient care needs and issues.
Job Responsibilities- Perform administrative tasks for the home health team managers, including electronic medical record tasks to improve timely patient care coordination.
- Serve as a liaison between nursing staff and patients and providers to ensure effective communication and delivery of care provided by the home health staff.
- Provide telephone triage to assess patient needs and provide care advice or education.
- Coordinate with labs and other departments to order diagnostic tests and relay results to providers as needed.
- Provide patient education on treatment plans, medications, and at-home care practices.
- Position works independently with several cross-functional departments and contacts including clinical management and support staff.
- Adheres to high standards of ethics and quality, Baystate policies and procedures, home health federal and state regulations.
- Performs other duties as assigned.
- Strong interpersonal and organizational skills with ability to manage multiple projects simultaneously.
- Excellent oral and written communication skills.
- Ability to work independently.
- Understanding of home health and hospice operations, or similar healthcare service lines.
- Comprehensive computer skills including electronic medical record systems and Microsoft Word, Excel, and Outlook.
Diploma or Certificate from an accredited practical nursing program.
Diploma in Nursing (Required).
Required ExperienceTwo (2) years of healthcare experience.
Preferred ExperienceTwo (2) years of home health experience.
CertificationsUnencumbered Massachusetts nursing license;
Driver's License.
Registered Nurse - State of Massachusetts;
Driver License - Other.
