A $500 sign-on bonus is available ($250 after first week of training and $250 after successfully completing the first 90 days).
Position:
Rehabilitation Specialist - Full Time
Location:
Springfield, MA
Hours:
40 hours per week
Rate of Pay: $26.00 per hour
Schedule:
Monday - Friday 8:00am - 4:00pm
- Gather data, document and present each member’s goals and objectives in accordance with ISP regulations.
- Coordinate services with case managers and parent/guardians of program members’ goals, objectives, issues, concerns, and abilities.
- Ensure the provision of high-quality services.
- Oversee the screening and prioritizing of referrals; document member progress; implement behavior management strategies.
- Investigate incidents and perform all other duties as assigned.
- High School diploma with 5 years related experience OR Associate’s degree with three years related experience OR Bachelor’s degree with one year related experience.
- Knowledge of and respect for diverse populations. Previous experience working in human services preferred.
- Requirements: 2 years of previous case management or care management experience. Management experience required.
- Ability to use computers to include Microsoft Word, data entry, email, and internet;
Credible experience preferred. - Must be organized, detail-oriented, with solid time management skills and the ability to work in a fast-paced, multi-tasking environment.
- Demonstrated ability to deliver high levels of customer service with both internal and external customers.
- Excellent communication, interpersonal and listening skills. Must have a valid driver’s license, good driving record, use of a vehicle and willingness to transport participants in a personal and/or agency vehicle.
