Qualifications
  • Doctor of Pharmacy degree.
  • 1-3 years of experience.
  • Massachusetts License or Training/equivalent experience in Medication Therapy Management (MTM).
  • CPR Certification.
  • Detail oriented.
  • Ability to interact with diplomacy and tact.
  • Ability to work with people from a variety of culturally diverse backgrounds.
  • Reliable and professional in behavior and appearance.
  • Must be flexible to work including Saturdays.
Principle Responsibilities and Duties
  • Perform MTM in-person and telephonic pharmacotherapy consultations to patients and/or prescribers, promoting high quality, evidence-based, cost-effective pharmaceutical care.
  • Complete a comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities.
  • Complete pharmacotherapy assessment of patient terms such as claims, past medical history, and disease states.
  • Maintain a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric populations; serve as an authoritative information source on drugs and their utilization in therapy.
  • Assess patients for medication adherence barriers and develop a plan to maximize adherence to medications.
  • Develop a plan of care detailing actions of the patient and/or prescriber based on pharmacotherapy assessment.
  • Deliver high quality evidence-based therapeutic recommendations to providers.
  • Educate patients on their medications and promote effective medication use.
  • Document clinical consultations including the plan of care into internal database.
  • Participate in continuing education sessions provided by the department.
  • Actively participate in staff meetings, staff development opportunities, and interdepartmental work groups.
  • Identify areas for improvement where systems modifications could result in operational advancements and efficiencies.
  • Adhere to all departmental and organizational policies and procedures, as well as applicable laws and regulations.
  • Assist in outpatient pharmacy as needed.
