Minimum Requirements
  • Licensed as a LICSW, LMHC, or LMFT in the state of Massachusetts.
  • Strong professional license standing in the State of Massachusetts.
  • Ability to work independently and collaboratively with the Integrated Care Team.
  • Familiarity with multicultural groups and prior experience in a multicultural setting.
  • Prior experience addressing behavioral health needs of patients with chronic medical conditions (e.g., diabetes, hypertension).
  • Experience with quality improvement processes, tracking, and reporting on behavioral health indicators.
  • Strong interpersonal, written, and oral communication skills; proven ability to communicate with diverse audiences.
  • Demonstrated professionalism and respect, with a strong regard for patient privacy.
  • Excellent organizational skills.
  • Computer literacy.
  • Experience using an Electronic Health Record (preferable).
  • Bi‑lingual proficiency strongly preferred but not required.
  • Experience facilitating group therapy is a plus.
Key Responsibilities
  • Provide therapeutic interventions—including individual, group, family, couple, and consultation—for adults and children.
  • Offer Behavioral Health Screening, Brief Intervention, Referral, and Treatment services to CHC patients.
  • Coordinate and collaborate with the primary care team (providers, pharmacists, 操作人员) to improve health outcomes.
  • Provide a "warm hand‑off" for identified patients when schedule allows.
  • Maintain professional, accurate, and timely clinical documentation of patient encounters and services, and track behavioral health indicators as defined in the BHI grant.
  • Attend behavioral health meetings weekly and other designated meetings related to patient care.
  • Participate in supervision with the Chief of Behavioral Health as scheduled.
  • Perform other duties as assigned to support the mission of the organization.

Salary Range

$33.00 – $44.35 per hour.
