The Outreach Worker will work within a dynamic, multidisciplinary ACCS team model, serving as the primary caseworker for multiple participants. The role involves providing direct services to individuals with mental health diagnoses who live independently in the community. Responsibilities include engaging individuals to maintain their physical and mental health, supporting activities of daily living, administering medications as needed, advocating for clients, and offering opportunities to enhance independence, rehabilitation, and recovery.
Requirements
- Must be at least 21 years old.
- Preferably have or be working towards an Associate’s or Bachelor’s degree.
- Basic computer skills are required.
- Must have a vehicle for work, a valid driver’s license, and complete medication administration training upon hire.
- All candidates must pass a background screening process.
