Escorts patients to different programs within the hospital and community to connect with recovery resources.
Works with patients and providers to set care goals and motivates patients to achieve them.
Teaches key educational messages in person and over the phone, and clearly documents all activities in the patient’s record.
Records and monitors participants’ progress toward goals within specified time frames.
Participates with staff in activities such as community outreach, presentations, development of materials, and phone calls.
Assists patients with organizing records, making follow-up appointments, and filling prescriptions.
Helps patients complete applications as needed.
Provides advocacy, education, and support in accessing community-based and hospital programs.
Refers patients to internal or external care management services when other issues are identified (e.g., food insecurity, partner violence).
Demonstrates sensitivity and respect for patients' cultures.
Participates in regular training activities and attends scheduled supervision and meetings.
Influences appropriate utilization of healthcare resources through outreach, motivational interviewing, goal setting, and accompaniment to medical appointments.
Presents patient cases succinctly at review meetings and consults with experts on complex situations to develop optimal care plans.
Prepares reports and documents as needed or requested.
Functions effectively within an interdisciplinary team, connecting patients with resources as needed.
Supports the Operations Manager in implementing and evaluating quality initiatives.
May visit patients in hospitals and ERs to facilitate care transitions.
