Schedule:
- 40-hours
- Day shift
- Weekend rotation every 3-4 months
- Holiday rotation every 1-2 years

- Master’s in Social Work (required)
- LCSW or LICSW active in the State of Massachusetts (required)
- Minimum of three years as a practicing social worker in a healthcare or social service setting (may include time in an equivalent Master’s level internship)
Summary
Principle duties related to direct clinical services. Address psychosocial issues that arise in acute care, emergency department and/or inpatient settings through focused Social Work interventions. Services are provided upon referral by nursing staff/physicians, community contacts, or by patients or their families. Address psychosocial functioning that affects or is affected by the patient’s medical condition, coping skills, and discharge status. Interventions include direct counseling of patients and families regarding emotional, financial, environmental and safety issues, as well as referrals and coordination with community agencies.
The role supports continuity of care and optimal outcomes in the inpatient setting.
Demonstrates knowledge about human behavior and the psychosocial impact of disease across ages; coordinates input from health care professionals, patients and families to formulate discharge plans. May include collaboration, and/or education, with other health care team members. Demonstrates clear written and verbal communication skills. Supports the mission and operating standards of Baystate Health.
