  • 40-hours
  • Day shift
  • Weekend rotation every 3-4 months
  • Holiday rotation every 1-2 years

Qualifications
  • Master’s in Social Work (required)
  • LCSW or LICSW active in the State of Massachusetts (required)
  • Minimum of three years as a practicing social worker in a healthcare or social service setting (may include time in an equivalent Master’s level internship)

Summary

Principle duties related to direct clinical services. Address psychosocial issues that arise in acute care, emergency department and/or inpatient settings through focused Social Work interventions. Services are provided upon referral by nursing staff/physicians, community contacts, or by patients or their families. Address psychosocial functioning that affects or is affected by the patient’s medical condition, coping skills, and discharge status. Interventions include direct counseling of patients and families regarding emotional, financial, environmental and safety issues, as well as referrals and coordination with community agencies.

The role supports continuity of care and optimal outcomes in the inpatient setting.

Demonstrates knowledge about human behavior and the psychosocial impact of disease across ages; coordinates input from health care professionals, patients and families to formulate discharge plans. May include collaboration, and/or education, with other health care team members. Demonstrates clear written and verbal communication skills. Supports the mission and operating standards of Baystate Health.

Job Requirements
  • Provides a comprehensive age-specific psychosocial assessment and develops a psychosocial treatment plan; includes cultural and spiritual variables. Assesses risk of abuse/neglect and files reports as needed.
  • Formulates treatment goals/plans with the patient and refers to other disciplines as needed. Respects patient rights within psychosocial, spiritual, developmental and cultural contexts.
  • Provides counseling to patients and families using diverse techniques. Stays current in social work practice trends and maintains strong social work skills.
  • Participates on the care management team to ensure timely post-hospital care plans; documents transition of care and revises plans as indicated. Makes referrals to social services/community agencies as needed and communicates with the health care team, patient and family throughout the process.
  • Works with hospital attorneys on legal issues such as guardianship or commitment for individuals with substance use disorders. Performs DSM multiaxial assessment as needed and coordinates referrals to mental health services as appropriate.
  • Maintains records and adheres to documentation policies; understands Electronic Health Record systems; documents on-call interventions as indicated.
  • Adheres to department policies and productivity standards; participates in quality improvement activities; supports weekend and special coverage needs as required.
