Job Overview

Family Med Practitioner – Caring Health Center, Inc., Springfield, MA.

Full‑time position.

• Provide direct patient care services within area of expertise.
• Assess, plan, document, and evaluate patient needs and treatments; ensure appropriate follow‑up.
• Maintain a practice that is culturally and linguistically sensitive, using interpreters and other assistance as appropriate.
• Design a plan of care based on assessment of physical, emotional, and social needs, in collaboration with the patient and family when appropriate.
• Refer patients to specialists, other practitioners, and facilities as needed.
• Review patient charts and ensure that appropriate treatment plans are developed and followed.
• Collaborate with other caregivers regarding patient education and rehabilitation, including promotion of mental and physical health.
• Serve on-call rotation as required.
• Cooperate with CHC administration to monitor and improve the quality of patient care provided.

• Unrestricted Massachusetts medical license.
• M.D. degree.
• Three years of postgraduate residency training in an accredited Family Medicine residency program.
• Board eligibility in Family Medicine.
• Current DEA and Massachusetts Controlled Substances registrations.
• Ability to provide quality care in a high‑volume, multicultural Health Center setting.
• Unrestricted admitting privileges at Baystate Medical Center and Mercy Medical Center, maintained during tenure.
• Provider status for major insurers.
• Board Certification in Family Medicine (desired).
• Fluency in Spanish, Russian, or Vietnamese (desired).
• Special expertise in providing health care to underserved, culturally diverse populations.
