The Home Health RN provides nursing care to assigned patients, coordinating care, supervising care by Home Health Aides, and adhering to nursing policies and procedures. The role requires full responsibility for patient care and collaboration with other disciplines to ensure optimal outcomes.

• RN license required; BSN preferred.
• Current CPR certification required.
• Valid Illinois driver’s license, acceptable driving record, and access to a personal vehicle with proof of insurance.

• Minimum of two years acute care clinical experience preferred; home health experience is a plus.

• Perform age-appropriate assessments and develop individualized care plans.
• Function as a case manager, providing after-hours care on a rotation basis.
• Supervise LPNs and HHAs, ensuring quality care and documentation.
• Communicate effectively with patients, families, and interdisciplinary teams.
• Participate in educational and quality improvement initiatives.
