Overview
The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific care to primary care patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting.
DutiesOne vacancy will be located at Springfield CBOC and one vacancy will be located at Decatur CBOC. The PACT RN assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The RN, PACT, performs duties that include, but are not limited to the following:
