Home Health Occupational Therapist – $10,000 First-Year Success Bonus
Graham Healthcare Group is seeking a Home Health Occupational Therapist for a job in Springfield, Illinois.
Responsibilities- Collaborate with the care team to manage and deliver patient care plans.
- Support patients in Springfield and surrounding areas.
- Assist in the day‑to‑day delivery of care and monitoring of patient progress.
- Must possess a Bachelor of Science or Master's degree in Occupational Therapy from a school accredited by the Council of Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association.
- Licensed Occupational Therapist issued by the state in which you will be working.
- Minimum of one year healthcare experience.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- Commitment to consistently meet critical deadlines for charting.
- Self‑manage your time and schedule.
- Demonstrated experience with tablets, mobile phones andR software.
- Successful completion of a drug screen prior to employment is part of the background process.
- Confirms legal authorization to work in the United States.
