Graham Healthcare Group is seeking a Home Health Occupational Therapist for a job in Springfield, Illinois.

• Collaborate with the care team to manage and deliver patient care plans.
• Support patients in Springfield and surrounding areas.
• Assist in the day‑to‑day delivery of care and monitoring of patient progress.

• Must possess a Bachelor of Science or Master's degree in Occupational Therapy from a school accredited by the Council of Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association.
• Licensed Occupational Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Self‑manage your time and schedule.
• Demonstrated experience with tablets, mobile phones andR software.
• Successful completion of a drug screen prior to employment is part of the background process.
• Confirms legal authorization to work in the United States.
