• Conduct pre-admission interviews and assessments to gather essential information and develop individualized care plans
• Collaborate with interdisciplinary team members to ensure a comprehensive understanding of resident needs and develop effective care strategies
• Coordinate necessary documentation, paperwork, and administrative tasks to facilitate the admission process
• Develop and maintain relationships with referral sources, including hospitals, rehabilitation centers, and other healthcare organizations
• Provide exceptional customer service to residents, families, and referral sources, ensuring a high level of satisfaction and loyalty
• Maintain confidentiality and adhere to HIPAA regulations at all times
