PT Home Health Springfield, IL
As a Physical Therapist on our client’s Home Health team, you’ll deliver high-quality physical therapy services to patients in their homes. This role involves evaluating functional abilities, developing individualized treatment plans, and helping patients improve independence as well as physical and cognitive functioning. You’ll also provide education and guidance to patients and caregivers, oversee appropriate supervision, and document patient progress throughout the course of care.
New graduates are encouraged to apply and will receive support and mentorship to grow in the role.
- Perform Physical Therapy assessments and develop treatment programs
- Record and report patient’s reaction to treatment and condition changes
- Instruct patients and caregivers in therapy programs and device use
- Supervise physical therapy assistant and, when appropriate, the certified nursing assistant
- Prepare reports and participate in staff and agency development activities

- Licensed Physical Therapist issued by the state in which you will be working.

- A commitment to consistently meet critical deadlines for charting
- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
