RN - Admissions/Liaison - Full Time

Circle of Life is seeking an RN to provide admissions liaison services to patients and families in the Northwest and Northeast Oklahoma region. This role includes nights, days, and weekend shifts.

The Admission/Liaison Nurse is an integral part of the Interdisciplinary Team and works closely with the Medical Director to facilitate hospice admissions. The nurse will coordinate and execute the admission process into hospice care, communicate the hospice concept to families and patients, develop care plans, train and educate patient/caregivers, and establish appropriate discipline frequencies to meet patient/family needs. The position requires flexibility, readiness to adjust schedules on short notice, daily travel, and strong clinical skills including head‑to‑toe assessment, patient education, physician consultation, and order execution.

• Current unrestricted AR RN license
• BLS certification
• Strong assessment skills with critical thinking and problem‑solving in a healthcare setting
• Strong documentation and communication skills, ability to remain calm during human suffering and emergencies
• Hospice, home health, or acute care nursing experience preferred; computer charting experience desirable
• Personal vehicle, valid driver's license, and current auto insurance
