Position: 104 PASRR Assessment Specialist

West Texas Centers is seeking a PASRR Assessment Specialist to perform Pre-Admission Screening and Resident Reviews (PASRR) to determine the appropriateness of nursing facility placements and identify eligibility for specialized services.

This position is responsible for coordinating and conducting PASRR assessments for individuals identified through the LTC Online Portal who have a mental illness, intellectual disability, or developmental disability and are either (1) seeking admission to a nursing facility, or (2) currently residing in a nursing facility and experiencing a significant change in status.

The role includes interviewing individuals, their LARs, and other collaterals; completing and entering assessments into TMHP within required time frames; and participating in interdisciplinary team (IDT) meetings for individuals who qualify for specialized services. Extensive travel throughout the WTC service area is required.

Starting Salary $21.84

Responsibilities:
  • Monitor the LTC Portal daily for alerts.
  • Conduct or coordinate face-to-face assessments within 72 hours of identification.
  • Complete PASRR assessments (Expedited, Exempted, Pre-Admission) within prescribed timelines.
  • Enter assessments into TMHP within required time frames.
  • Maintain and update administrative documentation in the EMR.
  • Provide notifications to nursing facilities and individuals regarding eligibility; complete Medicaid Fair Hearing letters.
  • Participate in IDT meetings for persons meeting criteria for specialized services.
Position Requirements :
  • Required:

    Bachelor's or advanced degree from an accredited college or university
  • Must have and maintain a Valid Drivers License and driving record that is acceptable for insurability through our insurance carrier.
  • Must pass background check and drug screening.
  • Applicants must reside within 70 mile radius of duty site
