• Fort Bend HC – Rosenberg

• Laurel Court – Alvin
• The Colonnades at Reflection Bay – Pearland

• Screens, evaluates, and treats patients.
• Completes a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
• Reviews patient records for current diagnoses, past medical history, precautions, and contraindications.
• Establishes measurable goals and develops a treatment plan to move the patient toward his or her maximum level of independence and/or function.
• Supervises physical therapist assistants and rehab techs in direct patient care and patient‑related activities in accordance with state practice act.
• Interprets and communicates evaluation findings and treatment program to patients, family, and other team members.

• Graduate from an accredited School of Physical Therapy.
• Current licensure as a physical therapist in the state of current practice.
• Must have good communication skills and be able to relate professionally and positively to patients, patients’ family members, and staff.
• Must be capable of performing the responsibilities of this job, with or without reasonable accommodations.
