Intake Coordinator
Spring Valley, NY
$50,000-$65,000
The Intake Coordinator is the initial point of contact for services within the agency. The Intake Coordinator assists the enrollee/family to explore which services the enrollee would be eligible for and refer the enrollee to the appropriate program. The intake coordinator also works with the client/family to apply for necessary programs.
KEY RESPONSIBILITIES
- Serve as initial point of contact for potential clients and their families, responding to inquiries and providing information about the company's services, policies, and procedures.
- Conduct comprehensive assessment of clients' needs, including physical, emotional, and social requirements, to determine the appropriate level of care and support.
- Gather necessary information during the intake process, including medical history, current medications, allergies, and any specific care preferences and requirements.
- Collaborate with clients, their families, and nursing team staff to develop personalized care plans that alight with the client's goals and needs.
- Assist current clients with applying for additional services when eligible.
- Maintain accurate and up-to-date client files, including insurance information, consent forms, and other relevant documentation.
- Establish and maintain positive and professional relationships with clients, their families, and other stakeholders, demonstrating empathy, active listening, and effective communication skills.
- Ensure compliance with the regulatory requirements and company policies related to client records and confidentiality of information.
- Participate in regular meetings and training sessions to stay updated on industry trends, best practices, and changes in regulations.
- Other duties as assigned.
- High School Diploma
- At least 1 year of work experience.
- Strong communication skills with the ability to engage patients warmly and professionally.
- Excellent problem-solving skills.
- Organized, detail-oriented, and able to manage multiple priorities.
- Bilingual in Yiddish a plus
