Responsibilities:
Academic Preparation:
* Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO)
* Minimum ADN preparation with expectation of BSN completion within 5 years of hire
* BLS within 30 days of hire
* Responsible for walk in and telephone triage.
* Coordinates patients care through collaboration with the patient/family and healthcare team.
* Provides patient care by assessing, planning, implementing, evaluating and documenting individual patient needs, actions taken and patient responses.
* Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc.
* Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task.
* Documentation within scope of practice.
* Performs other functions as required.
