Avalon Care Center at Northpointe in Spokane is seeking an Admissions/Wound Nurse to join our team in a dual role!

$43.00 - $48.95/hr

Sunday - Thursday: 12:00 pm - 8:00 pm

Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.

Responsibilities Admissions Process
  • Receive leads from providers, specialty physicians, county offices on Aging and Veteran service organizations, facilities, or any customer source and convert appropriate inquiries to admissions.
  • Coordinate inquiries from all hospitals in the market.
  • In conjunction with facility requirements/abilities, clinically access patients to determine if the patient meets admission criteria.
  • Make contact with potential inquiries as a representative of Avalon Health Care. Review charts, and assist with any questions from the case management team, referral source and/or family members.
  • Obtain the necessary information (medical and financial) to qualify patients for admission.
  • Facilitate patient placement based on clinical services needed, bed availability, patient/family preference, geographic location, etc.
  • Troubleshoot areas of Admission, Public Relations, pre-certification and physician follow-up in conjunction with facility.
  • Conduct needed assessments.
Wound Care
  • Provide input for individualized plan of care in collaboration with the resident/responsible party and interdisciplinary care team by documenting problems, statements, goals, and approaches.
  • Define realistic and obtainable long and short-term goals for the residents.
  • Initiate appropriate nursing interventions with time frames for outcome evaluations and document appropriately in medical record.
  • Revise plan of care with input from resident/responsible party and interdisciplinary care team.
  • Implement teaching with resident/responsible party consistent with interdisciplinary plan of care.
  • Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
  • Transcribe physician and/or other health care professionals’ orders appropriately and accurately.
  • Ask for clarification of questionable orders from the ordering physician and/or other health care professionals.
  • Notify physician and/or other health care professionals if orders are not carried out and document event appropriately.
  • Perform only those activities which are within the scope of LPN/LVN nursing practice.
  • Follow company, facility and departmental policies and procedures, and appropriately utilize nursing judgment.
  • Ensure accurate documentation of all medical records and reporting forms.
  • Promptly respond to call lights and other resident needs.
Qualifications
  • Associate and/or Bachelor’s Degree in health care field. Preferably Nursing.
  • Preferably 1-3 years of clinical nursing experience with at least two years in long term care and two years of successful experience in administration, consultation or involvement in the survey process.
  • Customer service, sales and marketing experience preferred.
Licenses / Certification
  • Must have an LPN in good standing.
  • Must meet all local health requirements for entering the hospital settings. As necessary, any drug screenings, tests, lab work or physical examination. If required.
