Duties include but are not limited to:
* Provide comprehensive nursing care to Veterans in a long-term care, rehabilitation, or hospice setting.
* Perform initial and ongoing assessments, including physical, emotional, and psychosocial evaluations.
* Develop, implement, and update individualized care plans in collaboration with interdisciplinary team members.
* Administer medications and treatments accurately and safely, following VA policies and protocols.
* Monitor and document vital signs, symptoms, and patient progress in the electronic health record (EHR).
* Respond promptly to changes in patient condition, initiating appropriate interventions and notifying providers.
* Support activities of daily living (ADLs) and promote independence and dignity for residents.
* Provide end-of-life care and emotional support to Veterans and families in hospice situations.
* Educate Veterans and families on health conditions, medications, and care plans.
* Maintain infection control standards and ensure a safe environment for residents and staff.
* Participate in quality improvement initiatives and adhere to VA performance measures.
* Collaborate with physicians, therapists, social workers, and other team members to optimize Veteran outcomes.
* Serve as a patient advocate, ensuring Veterans' rights and preferences are respected.
* Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
* Plans and delivers technically competent care to patients in alignment with the ANA Standards of Nursing Practice.
* Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks ensuring assigned delegated tasks are completed by team members.
* Manages nursing care of patients, organizes, and completes own assignments in a safe and timely manner, assisting team members as necessary.
* Use of Electronic Health Record.

Pay:
Competitive salary, regular salary increases, potential for performance awards
