Full Time – Permanent

Spokane, WA

(Will be covering the following County's: Ferry, Lincoln, Okanagan, Pend Oreille, Spokan, Stevens County's)

Monthly Salary Range: $6,043 - $8,123

The Department of Corrections is seeking a highly motivated and qualified Corrections Specialist 3 located in Spokane, WA.

This position serves a critical function of providing reentry services to a specialized caseload of individuals with mental health diagnoses and risk to community who are designated into the Reentry Community Services program pursuant to RCW 72.09.370. By addressing specialized needs as designees release from prison this position enhances public safety and positively supports individuals transitioning into the community.

This position prepares and maintains comprehensive transition plans which are essential for supporting patient wellness, reduction of recidivism, and reduction of emergent services utilization. Reentry Community Services is shown to be cost effective and reduce recidivism through the Washington State Institute for Public Policy. This program requires a multidisciplinary approach and individuals in this position act as a liaison for program participants, Prison and Reentry staff, representatives of outside organizations and/or members of the public in defining needs and planning.

Provide reentry planning for specialized caseload of individuals designated into the Reentry Community Services Program.

  • Establish and coordinate the Multisystem Care Planning Team.
  • Identify risk and assess treatment and community needs with corresponding resources in development of the comprehensive Transition Plan.
  • Complete transition planning process for program participants per RCS Protocols.
  • Facilitate transportation needs, may include transporting individuals releasing from prison with other DOC Staff.
  • Collaboration and partnership with team members.

Collaboration with internal and external stakeholders.

  • Consultation with Health Care Authority contracted RCS treatment providers during the reentry planning process.
  • Participate and provide recommendations in facility and community RCS case staffing’s.
  • Attend monthly staffing’s with community partners.
  • Develop and maintain working relationships with housing providers in assigned areas.
  • Participate in community resource partnership meetings.

Maintain documentation of pre and post reentry planning activities.

  • Complete and post RCS transition plans.
  • Document in-person and other contact with the designee and stakeholders.
  • Provide Advance Directive information to the designee per policy.
  • Monitors and tracks designated individuals’ participation in program activities.

Assist with transition of other high needs patients to the community

  • Collaborate with prison and community staff to assist in identification of resources available to individuals releasing to assigned sections.

Other duties as assigned

  • Participate in Health Services Quality Improvement Activities.
  • Complete annual in-service training as assigned.

Required qualifications:

  • Three to five years’ experience with case planning for justice involved individuals with behavioral health needs, documentation compliance, tracking and monitoring progress in real time and recording and reporting those interactions and meetings into a database.
  • Microsoft office knowledge and proficiency. Teams and other virtual platform experience and comfort with being on screen.
  • Excellent time management and record keeping skills.
  • Working knowledge of community mental health, housing and other reentry resources.

Preferred/Desired

Qualifications:

  • Bachelor's degree in Psychology, Social Work, Sociology or other relevant degrees within social services.

AND

  • At least one year experience within an institutional setting (i.e. prison, mental health, juvenile justice facilities).
