PATIENT CARE COORD - Per DiemJoin the Inland Northwest Behavioral Health team as a Patient Care Coordinator (PCC) on a per diem schedule. The PCC engages patients who may benefit from an Intensive Outpatient Program (IOP) and ensures they receive the appropriate referrals and support.
Responsibilities- Screen patients for IOP eligibility and review initial assessments.
- Collaborate with clinical staff to understand medical, psychological, and social needs.
- Schedule follow‑up appointments and maintain open communication among providers and patients.
- Educate patients and families about IOP structure, goals, and expectations.
- Connect patients to community resources such as support groups, housing and financial assistance.
- Monitor patient progress and adjust transition plans as necessary.
- Minimum 1 year of customer‑service experience in a medical setting.
- Agency‑affiliated counselor.
- Accreditation in BLS, First Aid, and Verbal De‑Escalation (provided by INBH).
- 2 years of experience in hospital, behavioral health, or medical setting.
- Licensure in Washington as LMFT, LICSW, LPCC, or LMHC.
