Overview

A Primary Care Physician leads a multidisciplinary team to provide comprehensive, value-based care focused on seniors' physical, emotional, and social wellness. They evaluate and treat patients, coordinate specialty referrals and care services, and ensure documentation and quality standards compliance. This role emphasizes personalized, concierge-level care to improve health outcomes in a senior-focused primary care environment.

Centers support patients, families, and associates with a collaborative culture and opportunities for growth. Center Well emphasizes coordinated, analytics-supported care and deep community relationships to improve outcomes.

Responsibilities
  • Evaluates and treats center patients in accordance with standards of care.
  • Follows appropriate level of medical care and quality for patients and monitors care using available data and chart reviews.
  • Assists in coordinating patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
  • Acts as an active participant and key source of medical expertise with the care team through daily huddles.
  • Supports Regional Medical Director and Center Administrator in setting a cooperative, professional, and approachable practice tone.
  • Completes all medical record documentation in a timely manner, working with a quality-based coder to optimize coding specificity.
  • Follows policy and protocol defined by Clinical Leadership.
  • Meets with the Regional Medical Director about quality of care, outcome data, policy, procedure, and records issues.
  • Participates in growth opportunities for new or existing services within the Center.
  • Participates in the local primary care “on-call” program of Center Well as needed.
  • Ensures compliance with licensing, certification, and accrediting bodies.
  • Spends 100% of time clinically focused on direct patient care, including patient-facing time and related administrative tasks.
Required Qualifications
  • Graduate of an accredited MD or DO program and holder of an unrestricted medical license in the practicing state; eligible and willing to obtain licenses in other states in the region as required.
  • Experience with integrated, team-based care; strong interpersonal and communication skills.
  • Basic computer skills, including email and EMR.
  • Excellent verbal and written communication skills.
  • Demonstrated commitment to superior patient/customer service.
  • Willingness and ability to learn and adapt to a value-based care setting.
  • Active in TB screening program; must undergo TB screening if selected.
Preferred Qualifications
  • Board Certification or Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA) preferred.
  • At least 2 years of relevant experience.
  • Active and unrestricted DEA license.
  • Medicare Provider Number;
    Medicaid Provider Number.
  • Experience managing Medicare Advantage patients and knowledge of coordinated care in value-based healthcare.
  • Knowledge of Medicare guidelines, coverage, and HEDIS quality indicators.
  • Bilingual is a plus.
