Seeking a Full-Time Physical Therapist Assistant (PTA) to join the in-house therapy team at Southport Nursing and Rehab in Southport, CT.

Located at 930 Mill Hill Ter, Southport, CT 06890

The primary responsibility of the Physical Therapist Assistant (PTA) is to provide direct physical therapy interventions as planned, delegated, and supervised by the Physical Therapist. The role includes recording treatments and noting progress in medical records, ensuring high-quality care for residents.

Resident Care:

  • Provide individualized treatment plans and updates to increase residents’ independence and functioning under supervision of a Licensed Physical Therapist and in accordance with physician’s orders.
  • Communicate changes in residents’ conditions.
  • Maintain accurate billing records and clinical documentation.
  • Coordinate and provide training with Restorative Nursing and other Care staff.
  • Provide education and training to patients, caregivers, and community partners.
  • Ensure compliance with mandatory education, training, and competence programs.
  • Perform screening rounds as directed by the Director of Rehabilitation to assess client needs and environmental safety.
  • Provide recommendations for business improvement, safety concerns, or updates to the Director of Rehab and team members.
  • Prioritize and multi-task effectively.
  • Be detail-oriented and follow through on tasks.
  • Manage time efficiently and work well with staff, residents, and families.
  • Solve problems effectively.
