• Perform comprehensive assessments to establish goals for symptom management and end‑of‑life care using effective case management strategies.
• Participate as a member of an active interdisciplinary team that addresses patients’ needs holistically.
• Develop and maintain therapeutic relationships by utilizing effective communication with patients and their families.
• Utilize state‑of‑the‑art technology to document skilled services provided.

• Registered Nurse with a valid CT RN license.
• One year of RN experience; home‑care or hospice experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.
