Position: Supports Coordinator I

Overview

The Guidance Center provides a broad range of services that have helped children, adults and families unlock potential and build better lives since 1958. With 25 programs we offer treatment, prevention, growth and education services to more than 18,000 people annually. The Supports Coordinator is responsible for ensuring all service needs of a person receiving services are met by providing linking, coordinating, monitoring, advocating, negotiating and referral services through the provision of supports coordination services. The Supports Coordinator is responsible for the assurance of the creation of the Person Centered Plan, completing appropriate documentation including assessments other than Integrated Health Assessments, as well as monitoring of the established goals and the implementation of various treatment plans.

Regular working hours are expected to be Monday through Friday 8:30 – 5:00 with four (4) hours per week after 5 p.m. Due to the needs of the person receiving services and the program these hours may be adjusted to include weekend hours.

We have a market-based compensation structure. The estimated salary for this position will vary based on a candidate s transferable work experience and qualifications. Hourly rates start at $23.82 for this role. Responsibilities Include

  • Develops timely and complete individualized plans of service based on the comprehensive assessment which includes goals established by all appropriate disciplines
  • Coordinates and completes all necessary referrals on behalf of client
  • Completes all required paperwork within program standards and expectations
  • Meets on-going productivity standards
  • Monitors all services provided on behalf of client
  • Monitors satisfaction with services
  • Provides family training and crisis intervention as appropriate
  • Completes peer audits as assigned and timely submits results to supervisor
  • Provides resource development and shares with peers in an attempt to reduce the amount of service gaps
  • Develops and maintain written records necessary for professional practice as defined by agency standards
  • Provides outreach services to individuals and their families in their respective locales and/or homes as appropriate to their unit’s function
  • Provides community education services as required
  • Participates in meetings as required including but not limited to: supervision, team meetings and case consultations
  • Follows all agency policies including but not limited to:
    The Guidance Center’s professional code of ethics
  • Represents The Guidance Center in the community
  • Must complete 24 hours of program and/or position specific training on an annual basis
  • Other duties as assigned

Requirements

  • Must possess a bachelor’s degree in a Human Services field including but not limited to Social Work, Counseling and Psychology
  • At least seven (7) months of experience in the area of intellectual and/or developmental disabilities is required
  • Must have MDHHS Central Registry and Police clearance
  • Home and/or community-based service experience is preferred
  • Valid chauffeur’s license and favorable driving record is required. Candidates who do not have a chauffeur s license may still apply but must acquire one by the end of their first month of employment
  • Additional requirements include: a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual families, the ability to engage and maintain clinical relationships with families, the ability to develop collaborative relationships with other providers, effective interpersonal communication skills, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative.
