Social Worker - Behavioral Health

Overview

TLC Nursing Associates, Inc. is seeking a compassionate and dedicated Social Worker - Behavioral Health to provide support, counseling, and case management services to individuals and families facing mental health and behavioral challenges. This role involves conducting assessments, developing care plans, and connecting clients with community resources to promote wellness and recovery.

• Conduct psychosocial assessments to identify client needs, strengths, and support systems.
• Develop and implement individualized care plans addressing behavioral health needs.
• Provide individual, group, and family counseling to support mental health and emotional well-being.
• Collaborate with psychiatrists, psychologists, nurses, and other healthcare professionals as part of a multidisciplinary team.
• Assist clients in accessing community resources such as housing, employment, financial aid, and support groups.
• Advocate for clients’ rights and ensure continuity of care across service providers.
• Maintain accurate, confidential documentation in compliance with legal and ethical standards.
• Provide crisis intervention and develop safety plans when necessary.

• Master’s degree in Social Work (MSW) required.
• State licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required.
• Experience in behavioral health, mental health, or counseling settings preferred.
• Knowledge of community resources, behavioral health interventions, and case management practices.
• Strong communication, empathy, and advocacy skills.
• Ability to manage crisis situations effectively and remain calm under pressure.
