Home Health Field Registered Nurse

We are looking for a compassionate RN to join our growing team! The right candidate has a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide medical support to our clients in their homes. Must know Kinnser/Well sky documentation.

• Graduate of an approved school of professional nursing and currently licensed in the state of Michigan
• One year of nursing experience
• Kinnser - EMR experience preferred
• Home Healthcare experience preferred
• Experience in wound vac, PICC line, and mediport care, preferred
• Pediatric experience, desired

• Provide services according to the plan of care.
• Conduct initial and regular reevaluations of the patient's nursing needs.
• Initiate and revise the plan of care as needed.
• Deliver services requiring specialized nursing skills.
• Implement preventive and rehabilitative nursing procedures.
• Document patient visits and care summaries promptly, following agency policy.
• Coordinate services and communicate patient condition changes.
• Counsel patients and families on nursing and related needs.
• Participate in and present in-service programs.
• Adhere to agency policies and procedures.
• Process orders, notify physicians of changes, and complete certifications and discharge summaries.
• Assess nursing needs for each patient individually.
• Refer patients requiring specialized therapy to appropriate professionals.
• Supervise and train nursing personnel.
• Conduct patient care conferences and participate in peer review and quality improvement activities.
• Participate in utilization review.
• Provide total patient care as needed.
• Take on-call duties during nights, weekends, and holidays.
• Complete and submit assessments, transfers, and discharge documentation timely, using ICD-10 codes.

Work indoors in the agency office and patient homes, with travel involved.
