Southend-On-Sea's Children Young People And Families Public Health Service service are looking to recruit an enthusiastic and motivated practitioner with initiative and drive. Our service delivers high quality public health nursing, that promotes and improves health inequalities to children, young people and their families in Southend-On-Sea. You will need to have excellent communication and organisational skills, be willing to facilitate and support change in service delivery, have the ability to work autonomously and as part of an integrated skill mix team.

Main duties of the job

You will work within a skill mix team, working closely with partners, such as our Health Visitors, School Nurses, early years, GPs and Social Care, providing seamless care to the children, young people and families of Southend. You'll also have an important role in recognising and protecting vulnerable children as well as using innovative ways to communicate within the Southend population.

The successful candidate will work within the Children Young People And Families Public Health Service, in the 0-5 health visiting team ensuring the delivery of safe, effective and high-quality services to a cohort of babies, children and families, using research-based evidence to inform practice with agreed professional standards. The post will require the practitioner to drive between homes, schools, and family centres within the Southend area.

Job responsibilities

The post holder will lead the provision of a family centred, evidence based public health service that promotes and improves health and addresses inequalities to a defined caseload, population or community.

This will be achieved through delivery of public health nursing model at a community, universal reach, targeted response and specialist response level. The post holder will work in line with the Local Authority corporate objectives and will be expected to make a positive contribution to the provision of public health services. The post holder will be expected to work in line with the Southend City Council values and behaviours delivering excellent care with compassion.

Experience
  • Effective oral / written communication and interpersonal skills.
  • Ability to work alone and as part of a team.
  • Skills in managing own workload through effective use of prioritisation.
  • Evidence of commitment to life-long learning.
  • Ability to travel extensively within acceptable time scales.
  • Experience of developing and managing groups.
  • Knowledge of Systm One.
  • An understanding of Strategic Direction of Childrens Services.
