IDT Nurse Assessor

Overview

We are currently looking for a bright, hardworking nurse to join our Integrated Discharge team at Southend Hospital to cover maternity leave until 16/08/2026.

The successful post holder will be responsible for the assessment of patients that are medically optimised for discharge to ascertain their care needs and make referrals for ongoing patient care in the community, to facilitate a safe and effective discharge from the acute trust in a timely manner.

• Excellent communication with patients, relatives, the MDT and with the wider system partners is key to facilitating safe discharges from the acute trust and aiding to maintain good patient flow throughout the acute trust.
• This role works across a 7 day period including Bank holidays. The Integrated Discharge Team works with patients and families with complex needs and conditions, supporting them in the discharge process in order for a smooth transition in to the community.
• The Integrated Discharge Team provides guidance and professional support to the patient and the patient s families, and works with them in partnership to make sure the patient gets the most appropriate plan in place for discharge, whilst ensuring that interventions are not duplicated. This process improves the quality of care as well as improving patient satisfaction.
• The Integrated Discharge Team support patients with complex healthcare needs, who may require long-term health and social care planning or a short term intervention following a change of condition or acute illness.
• We aim to anticipate, co-ordinate and join up health and social care needs for patients at a high risk of unplanned admissions to hospital.
• You will be at the centre of holistic care planning working within Southend Hospital alongside other organisations such as Social Care and the voluntary sector.
• You will be responsible for liaison and communication between hospital and all appropriate community services (informal and formal carers) in order to facilitate a smooth, safe and effective transfer from hospital to home and vice versa.
