Alvita Care is seeking experienced, Licensed Practical Nurse LPNs in Lichfield County. Reliable transportation preferred.

Details

• We have clients throughout CT and NY requiring shifts from 8hrs to 12hrs.
• Day and Night Shifts Available
• Work with us full-time or around your current work schedule
• Shifts are assigned to provide 1:1 care in the home environment

QUALIFICATIONS

• CT LPN licensure.
• BLS, ACLS, or CPR certification.
• Minimum of One (1) year demonstrated experience as an LPN.
• Ability to put patients at ease and explain physician s instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently.
• Valid driver s license or NYS Non-Driver photo , required.
• Drivers preferred.

Essential Duties & Responsibilities

• Consult with Management on complex/unique cases.
• Monitor/assess patients/clients  condition and the effects of the care plan.
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manage, plan, coordinate and execute the patient s safe discharge from service.
• Ensure physician s orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.

• Communicates and consults with patient s MD as needed; manage and coordinate all aspects of assigned caseload.
• Assist patients with activities of daily living.
• Document content and process of all services provided in internal system properly.
• Transport/utilize medical equipment to and from clients/patients  home/facilities. Travel to patient/clients  homes and/or other facilities.
• Perform other related duties and participate in special projects as assigned.
