Care Manager (CNA: PER Diem, all shifts, 6-2pm, 2-10pm, 10-6am)

Led by the community's Director of Health and Wellness, we are looking for compassionate and dedicated caregivers (Care Managers) to assist residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.
- Per Diem – All shifts available for Assisted Living and/or Memory Care
- Salary Range: $20.00 – $22.00 Hourly
- Provide resident care including bathing, dressing, toileting, and feeding as outlined in the care plan.
- Maintain the safety, comfort, dignity, and confidentiality of all residents.
- Encourage and escort residents to meals and activities when needed.
- Ensure all assigned residents are clean, neatly dressed, and groomed, and are fed during assigned shifts.
- Document all services provided to each resident during your shift.
- Monitor and report any change of condition in a resident immediately to your supervisor.
- Participate in and attend all in-service training as scheduled.
- Ensure compliance with all Federal, State, and local regulations, as well as community policies and procedures.
- High School Diploma or GED.
- At least 1 year of resident care experience required.
- Certified Nursing Assistant or Certified Home Health Aid.
- Knowledge and experience working with the elderly population; preferred dementia experience.
- The associate often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
- The associate must lift or carry up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. Proper body mechanics must be used.
